 Maternal Health & Well-Being

  • Mothers creating, and having an on-going, S.M.A.R.T. health care goal for themselves reached 74%*
  • 64% of mothers were screened for depression on time (a 34% increase over time)*
    • ​96% of those caregivers screening positive for depression were referred for depression treatment, discussed treatment options with their home visitor, were already receiving treatment, or were on a waitlist for services.
  • ​124 mothers were referred in 2020-2021 to supportive services through Moving Beyond Depression™
    • ​27 Mothers graduated from MBD, and of those graduates 89% had reduced depression symptoms and increased/sustained social support after participation*
  • 87% of caregivers discussed healthcare utilization with their home visitors*
  • 82% of caregivers discussed reproductive life planning options with their home visitors*

​ Child Health​ & Well-Being

  • Average breastfeeding duration - 11 months* with the percentage of mothers ever breastfeeding increasing by 3% from last year**
  • 82% of caregivers created, and had an on-going, S.M.A.R.T health care goal for their child* 
  • Of the ​infants that were born to families enrolled in home visiting prior to the infants birth: 93% were born at healthy birth weight, and 91% were full term births***
  • Promise 1000 home visitors encourage families to attend well child visits, and offer supportive coordination of care by attending well child visits with families (this effort was directly impacted by COVID-19)
    • ​59%  of children attended their AAP recommended well child visit this year

Child Development & School-Readiness

  • 1,390 child development and social-emotional development screens were completed*
    • Child development and growth screenings were completed on time for 64% of Promise 1000 infants and children*
    • Child social-emotional development and growth screenings were completed on time for 62% of Promise 1000 infants and children*
  • 127 parent-child interaction observational screens were completed*
    • ​COVID-19 impacted the ability to do these typically in-person observations easily, but home visitors made every attempt to get them accomplished.
  • Upon completion of the Promise 1000 program (age 3 years) home visitors are able to refer families to early education services and preschool

Home Safety & Family Resilience

  • Families were screened for home safety to promote safe living environments, safe sleep, and safe traveling this year (there was a 32% increase of on time screens from baseline)* 
  • Promise 1000 home visitors screen for protective factors to strengthen families and encourage positive parenting practices (there was a 35% increase from baseline)*
  • Families received on time screenings to help prevent family violence (there was a 33% increase from baseline)*
