2024-2025 Results

1,000 children and 854 families received over 5,999 Home Visits from 8 fully participating Home Visiting Programs! These programs are serving families in X Counties!
Promise 1000's referral system pushed out 1,700 referrals to 52 referral partners across 52 counties in Kansas & Missouri!
Promise 1000 encourages and incentivizes its fully participating home visiting programs to become a part of the medical home and provide wraparound supports to home visiting families! One way we do this is through encouraging the home visitor to attend well-child checks and prenatal healthcare visits with the family. After many years of supporting this effort, we completed a study focused on the importance of early childhood home visitor attendance at well child visits. This summer, Promise 1000 was honored to have an article detailing this study published in the Clinical Pediatrics Journal.

​Through this study, we learned that when a home visitor supports a family at even one well child visit that they are more likely to attend well child visits suggested by the American Academy of Pediatrics (AAP) afterwards.
​

We also learned many things through the qualitative study that involved home visiting families, medical providers that have had home visitors attend medical visits, and home visitors. We learned that families who have home visitors attend well-child visits have 1) improved communication and understanding (including remembering questions, interpretation, understanding the visit and goals better, etc.), 2) better facilitation of the visit, and 3) emotional support.

2024-2025 Trainings included: ASQ (Developmental Screening) & PICCOLO (Parent-Child Interaction Screening) Implementation, The Benefits of Infant Massage, and MBD's Parental Emotional Regulation & Strategies, Suicide Intervention, and Family Relationships & Supports Trainings.

 Maternal Health & Well-Being

  • 456 caregivers were screened for depression this fiscal year 
    • ​Caregivers screening positive for depression were referred for depression treatment, discussed treatment options with their home visitor, were already receiving treatment, or were on a waitlist for services.
  • 85% of mothers participating in Moving Beyond Depression (MBD)™​ Services no longer met criteria for Major Depressive Disorder.​
    • ​Mothers also experienced a drop in parenting stress after treatment and an increase in social support (and healthy relationships)
  • 81% of families had a health, well-being, educational or safety related goal developed alongside their Home Visitor
  • Caregivers discussed healthcare utilization with their home visitors
  • Caregivers explored reproductive life planning options with their home visitors

​ Child Health​ & Well-Being

  • ​85% of mothers who enrolled prenatally in home visitation attempted, or pursued, breastfeeding their children post-enrollment.
  • Caregivers created, and had an on-going, S.M.A.R.T health care goal for their child​
  • Promise 1000 home visitors encourage families to attend well child visits, and offer supportive coordination of care by attending well child visits with families
    • Internal studies have shown that families had increased well child check (WCC) schedule adherence if they experienced a home visitor attending at least ONE well child check with them at some point.
    • This was regardless of how many home visits they had or how many times a home visitor attended a WCC with them!
    • Based on a qualitative study's results, the home visitor attending WCCs with the family also improved communication and understanding (including remembering questions, interpretation, and understanding the visit and goals better), facilitation of the visit, and emotional support for families.

Child Development & School-Readiness

  • Children received development and social-emotional development screens and were referred to child developmental resources , if needed.
  • Parent-child interaction observational screens were completed with families.​
  • Around 94% of families report that someone in the household is reading, telling stories or singing to their child daily.
  • In 93% of postnatal home visits, caregivers were asked if they had concerns regarding their child's development, behavior, or learning.
  • Upon completion of the Promise 1000 program (age 3 years) home visitors are able to refer families to early education services and preschool

Home Safety & Family Resilience

  • Home Visiting families were screened for home safety to promote safe living environments, safe sleep, and safe traveling this year​.
  • Through the Safe Sleep Safe Babies project, 1634 families were educated on safe infant sleep and received a safe sleep space and materials to date!
    • 103 safe sleep professionals have been trained across 25 community partner organizations and multiple hospitals have now become Safe Sleep Certified through Cribs for Kids (https://cribsforkids.org/)​
    • Overall, Safe Sleep Safe Babies participants scores indicated a statistical increase in safe sleep knowledge following safe sleep training.
  • Promise 1000 home visitors screen for protective factors to strengthen families and encourage positive parenting practices.
  • Families received on time screenings to help prevent family violence.
  • Home Visitors talk to families and help them prepare their homes for their little ones and they provide supplies to help set the home environment up safely.
    • 97% of participating children did not have an injury-related visit to the Emergency Dept. (ED) and 0% of injury-related visits to the ED results in a fatality.
  • Around 92% of caregivers had continuous health insurance coverage for at least 6 consecutive months.​
