Brief Profile of Families at Enrollment

Mothers creating, and having an on-going, S.M.A.R.T. health care goal for themselves increased by 58% from the 16-17 fiscal year**
Mothers screened for depression on time increased by 11% from last year**
80 mothers were referred to supportive services through Moving Beyond Depression™
- 15 mothers graduated from the MBD program with 87% of them demonstrating reduced depression symptoms and increased interpersonal support along with 80% showing reduced parental stress*
84% of caregivers discussed healthcare utilization and reproductive life planning options with their home visitors*

Average breastfeeding duration - 9 months, 11 Days* with the percentage of mothers ever breastfeeding increasing 5% from last year**
83% of caregivers created, and had an on-going, S.M.A.R.T health care goal for their child*
Of the infants that were born to families enrolled in home visiting prior to the infants birth: 93% were born at healthy birth weight, and 91% were full term births***
Promise 1000 home visitors encourage families to attend well child visits and offer supportive coordination of care by attending well child visits with families
- Families increased their on schedule attendance at well-child checks 3% since last year**

Infants and children received child development and growth screenings (a 44% increase in on time completed screens was achieved since last year)**
Infants and children additionally received child social-emotional development and growth screenings (a 33% increase in on time screens was achieved since last year)**
Upon completion of the Promise 1000 program (age 3 years) home visitors are able to refer families to early education services and preschool.

70% of families were screened on time for home safety to promote safe living environments, safe sleep, and safe traveling last year* (an increase of 30% from this time last year)**
Promise 1000 home visitors screen for protective factors to strengthen families and encourage positive parenting practices (a 28% increase in on time completed screens was achieved since last year)**
Families received on time screenings to help prevent family violence with a 25% increase in screens since last year**
